The Submit Tax ID link is an online form where you can provide your Social Security Number or EIN number.
It is a HIPPA-secure form. If you prefer you may download the IRS W-9 form to complete and then fax it to 1-307-333-7062.
Additionally please complete the Vendor payment form to help expedite the process.
