Please provide the following

• Income documentation from the last 30 days.
  • State if you have no income. 
  • SSI or SSDI = Current Benefit letter for 2026.
  • Paystubs - Monthly = 1 paystub, Bi-Weekly = 2 paystubs, Weekly = 4 paystubs.
• Income Certification from must be completed on file. If there is no income for any adult in the home this must be stated on this form. 
• A form of ID to establish identity.

Denials

• You may be denied if grant funds become unavailable.
• You may be denied if your income exceeds grant limits. 200% of federal poverty guidelines.
• You may be denied if you do not provide the required documents within 30 days of starting your registration.
