Eligibility Requirements
- Income must be at 125% of Federal Poverty Guidelines or less. May be up to 200% depending on status of Federal Continuing Resolutions.
- Income documents must be on file to support amount stated.
- Income documentation must include the last 30 days.
- Paystubs - Monthly = 1 paystub, Bi-Weekly = 2 paystubs, Weekly = 4 paystubs.
- SSI or SSDI = Current Benefit letter for 2025 or 2026.
- Income documentation must include the last 30 days.
- Income Certification from must be completed on file. If there is no income for any adult in the home this must be stated on this form.
- A resident of our service area. If no address is available, then other documents may be used to verify residency.
- A form of ID to establish identity. If ID is not available then another form of Identification may be used.
- Rent = Landlord payment information and landlord tax information (W-9). Lease, Amount due
- Utility = Utility bill - Must have your name and address, company name and address, your account number, date, and amount due.
- You may be denied if grant funds are unavailable.
- You may be denied if your income exceeds grant limits.
- You may be denied if you do not provide the required documents within 30 days of starting your application.
- You may be denied if you do not live in our service area being served by the program desired.
