Community Partners will act as the local contact for the participant and will communicate the status of our assistance provided that we have a release of information listing the community partner. Our process may take up to 15 days depending on delays encountered due to waiting for required information or documents. Please work with participants to have the following information to reduce times.

Our process may take up to 15 days depending on delays encountered due to waiting for required information or documents. Please work with participants to have the following information to reduce times.
- Legible Photo ID
- Signed Release of Information (Located under the FORMS page) automatically sent to us when completed.
- Verification of Residency (Any bill, paystub, ID, or lease that establishes their address)
- Last 30 days of income documents (Income documents must be legible and must contain the name of the party providing income, the disbursement time period, the participants name, and the gross income amount)
- If there is no income or an income document is missing the DECLARATION FORM (Located under the FORMS page) must be completed with a logical explanation as to why there is no income or a document is missing. The explanation must make sense and explain why there is no income, how they are surviving with no income, or why the document is missing.
- Documents supporting the payment to be made on behalf of the participant. That would include a bill, lease, or estimate. Documents must include the name of the party to be paid along with address and phone number. If there is an account number that must be present as well. Vendors may also complete the VENDOR PAYMENT form (Located on the Vendor Page under GET HELP)
- We will require a W-9 or tax ID information from every vendor to receive payment. We have this information for most vendors that we work with on a regular basis. If we do not have it on file we will reach out to them for the document.
- Narrative - We will work with you to identify the need of the participant and how our resource will support that need. Our organization works on an outcome basis which means that we provide resources as a means to achieve a specified outcome. We need to know that we will achieve a goal or an objective.
- Due to the amount of requests we receive participants are typically limited to one resource a year. There are some exceptions.
