Appeals Form

It is unfortunate, but sometimes we fail to meet your expectations. Please use the information below to follow our appeals process.
If you need to appeal a decision our staff have made, the following is our appeals process:
  • It is our policy that no person shall be excluded from participation, be denied benefits, or be subjected to discrimination in any activity or service.
  • It is our policy to not discriminate or to retaliate against anyone who files a grievance or cooperates in the investigation of a grievance. 
  • If the grievance involves a staff member under the supervision of a program director or the executive director then the grievance must be submitted to the executive director within 30 days the person filing the grievance becomes aware of the alleged action or instance of dissatisfaction.
  • If the grievance involves the executive director then the grievance must be submitted to the Board of Directors.
  • A complaint must be in writing. It must contain the name and address of the person filing it. The complaint must state the problem and the remedy sought.
  • Either the executive director or the board of directors will conduct an investigation of the complaint. Files and records will be maintained.
  • The investigating party will issue a written decision on the grievance no later than 30 days after its filing.
  • If investigated by the executive director the decision may be appealed to the board of directors.
  • If investigated by the board of directors and the grievance involves CSBG grant funds an appeal may be made in writing to the Wyoming Community Services Program within 30 days of the board decision. No request for a hearing from CSP may be made until heard by the board of directors.
